"In order to continue to provide the necessary attention and focus to patients, I have made a positive transition to a Concierge model of medicine. This yearly fee that is assessed allows me to focus my time on a much smaller number of patients in the office and in the hospital, with more flexibility and availability. This model of practice also allows for consultations over the phone and video conferencing during and after office hours. The enclosed documents provide further details and will hopefully answer many of your questions. Of course, I am happy to personally address any questions you may have regarding my new practice.

I will only be able to take a limited number of patients in order to make this model successful for both myself and for patients, so please notify my assistant Rachael at 312 922-1290 if you are interested and ready for this type of medical care.  After this limited number is met, I will then formulate a waiting list to work with as openings develop."
