David Lewis, DO

"Dr. David Lewis was born and raised in Port Angeles, WA. He attended the University of Washington where he received a Bachelor of Science degree in Biology in 2006. After undergraduate education, he worked one year with AmeriCorps before completing a Master of Arts in Education at Pacific Lutheran University. David then followed his lifelong goal of becoming a physician and attended Pacific Northwest University College of Osteopathic Medicine in Yakima, WA, where he met his wife Sarah. David attended residency at St. John Macomb / Oakland Hospital outside of Detroit, MI and completed 2 years of emergency medicine followed by 2 years of family practice. He is board certified in family medicine with special interests in cardiology, gastroenterology and dermatology.

David and Sarah then returned to Port Angeles, WA where they have been practicing for several years. Together they are raising their three children, Lydia (4), Matthew (2) and Evan (newborn). David enjoys the Pacific Northwest and the many outdoor activities it has to offer including hiking, camping and SCUBA diving. Additionally, he enjoys working on classic cars, collecting baseball cards and golfing."

"SEQUIM MEDICAL ASSOCIATES is 4-physician primary care practice in Sequim, Washington offering physicans who are board certified in Family and internal Medicine.   Located in the OMC Medical Services Building at 840 N. 5th on the second floor,  SMA provides easy access to ancillary services such as laboratory, medical imaging and various specialists. 

We provide 24-7 availability for our patients.  Our office is open Monday thru Friday.  You may contact us after hours, if an urgent need arises, by calling our office and reaching the answering service.  The answering service will then connect you directly to the physician on call if appropriate.  During office hours, we have dedicated staff for Coumadin management, referrals for specialized services, and assistance with your prescription needs.

We participate with most major insurance companies and manage all medical billing inhouse.  

We are a concierge practice.  Our patients pay a monthly fee based on age: $75 for those under 65 years of age, and $90 for those 65 and over.  The monthly fee provides services not billable to your insurance company.  These services include same day appointments when needed, less wait and unrushed visits, as well as quarterly newsletters and patient forums.  We are committed to providing easy access and pride ourselves on our personalized care and service."
