"Kinfolk Family Health in Montclair, New Jersey, is the heart and soul of Rakiyah Jones, DNP, FNP-BC, or Dr. Kai as his patients call him. Dr. Kai is a native of Newark, New Jersey. He is a nurse practitioner with a love for his community and a passion for community health.
At an early age, he envisioned bringing accessible care to urban communities leading to the birth of Kinfolk Family Health. At KinFolk Family Health he provides care to the insured and uninsured. For uninsured clients he offers membership-based individualized primary care, nutrition and mental health services. KinFolk Family Health specializes in integrative health and self-care focused wellness for People of Color, LGBT, Geriatrics and Veteran communities.
Dr. Kai is an Assistant Professor at Columbia University School of Nursing where he also completed his academic coursework, earning both a master’s degree in family nurse practitioner and palliative and end-of-life care, as well as his Doctorate in Nursing Practice (DNP). He is the Clinical Director of Transgender Health at Montefiore Medical Center in the Bronx. He completed a post master’s nurse practitioner certificate in adult-gerontology and acute care at the University of Pennsylvania in Philadelphia.
He served his country on active duty as a Commissioned Officer in the Army Nurse Corps and continues to serve as a Captain in the Army Reserves. Dr. Kai is truly committed to his patients and community and is dedicated to providing the education, support and holistic care these communities deserve."
