"Sara Poslaiko, DNP, APRN, FNP-BC, is a Doctor of Nursing Practitioner and Family Nurse Practitioner-BC, board certified through the American Nurses Credentialing Center. At Conviva Fort Pierce, she provides compassionate, patient-focused primary care with a commitment to medical excellence. Dr. Poslaiko earned a Doctor of Nursing Practice from Florida State University in Tallahassee, Florida. She obtained a Bachelor of Science in Nursing from University of Central Florida, Orlando, Florida; an Associate of Science in Nursing (Preceptorship as a surgical nurse in multi-disciplinary operating rooms) from Tallahassee Community College, Tallahassee, Florida; and an Associate in Arts from Miami-Dade College in Homestead, Florida. Her distinctions include the Award for Outstanding Scholarship of Practice by the Sigma Theta Tau International Honor Society of Nursing, the Most Outstanding Teaching Assistant by Florida State University; the Daisy Award by Tallahassee Memorial Healthcare; and Nurse of the Year by Select Specialty Hospital. She has served on the nursing faculty of Keiser University in Port St. Lucie, Florida, and as a registered nurse, teaching assistant at Florida State University in Tallahassee, Florida. She has served as a registered nurse in the cardiac ICU/CPCU of Tallahassee Memorial Hospital, and as a multi-hospital ICU/floor nurse, wound care nurse, and procedure nurse at Palm Beach Gardens Hospital in Palm Beach Gardens, Florida, Good Samaritan Hospital in West Palm Beach, Florida, Delray Medical Center in Delray Beach, Florida, and Select Specialty Hospital in Tallahassee, Florida. Her achievements include the DNP Project and her research, “Examining Psychosocial Factors that influence the transition processes from pediatric to adult healthcare in emerging adults living with diabetes between the ages of 18 and 30 years.” Dr. Poslaiko enjoys outdoor activities such as fishing, swimming, and bicycle riding."
